Telehealth Consent & Services
Virtual Care Limitations
You understand that our providers may not be able to treat all conditions via telehealth, and a physical diagnostic exam (such as palpation, auscultation, or rapid swabs) is not possible over video connection.
In-Person Care Referrals
If our providers determine that your safety or clinical symptoms require in-person care, you will be advised to seek in-person medical evaluation, laboratory testing, or emergency services.
No Guarantees
While we strive to provide excellent care, we cannot guarantee specific medical results, treatment outcomes, or prescriptions. All evaluations are subject to the independent clinical judgment of the provider.
You agree to provide truthful, complete, and accurate information regarding your medical history, current symptoms, active medications, and physical location (state). Providing false or misleading information can lead to improper clinical treatment, diagnostics errors, and the immediate termination of our services.
